what model number is this?

can anyone please tell me the model number of this? it has 1 cable in and 2 out...i know the 1 in 4 out is the tibbkf34 and the 1 in / 4 out ground block is the tidb34,i'm really looking for a ground block to match the 1 in/2 out block or a model number so i can search for it myself.....any help is much appreciated.
